Developmental Relationships Survey

Louisville City Schools will be conducting a survey of our youth in grades 7-12, called the Developmental Relationships Survey. It focuses on young people’s own sense of their relationships with others as well as skills that are essential for success in school and life. The information from this survey will help our district learn how to continue to improve upon the ways it supports young people. It does not ask more sensitive questions about potential high-risk behaviors.

  • The survey is not a test. It is a survey of your child’s experiences and opinions. There are no right or wrong answers.
  • There are no consequences if your child chooses not to participate.
  • The survey asks about young people’s own experiences and ideas as well as some information about themselves (e.g., age, gender, race-ethnicity).
  • There are no risks to participating in this survey. Your child will be told of their right to skip any question they do not want to answer.

If you DO NOT want your child to participate, you must complete and return this form by Monday, November 18. If no form is received, your child will be invited to participate. If you have any questions, please contact Mr. Faye at 330-875-1438.
