Emergency Medical Authorization
This form enables parents and guardians to authorize the provision of emergency treatment for children who become ill or injured while under school authority when parents or guardians cannot be reached. This form is required by law to be kept on file.
Free & Reduced Lunch Application
Children need healthy meals to learn. Louisville City School District offers healthy meals every school day. Breakfast is free district-wide! Lunch costs are K-5 ($2.50) 6-8 ($2.75) 9-12 ($3.00) Your child may qualify for free meals or reduced price meals. Reduced price is ($0.40) for lunch.
New Student Registration Form
This form needs to be completed in order to enroll a new student in the Louisville city School system.
Pay to Participate
All students who hope to participate in athletics, extra-curricular, and co-academic activities will be charged a Pay to Participate fee. All students who hope to participate in athletics, extra-curricular, and co-academic activities will be charged a Pay to Participate fee.
Student Computer & Internet Agreement
The District provides access to technology in order to enhance the instruction of its students and achieve the goals as established by the District Board of Education and the Ohio Department of Education. This Acceptable Use Policy (AUP) is established to promote the use of technology in a manner that leads to a safe and worthwhile educational experience for all.
