New Student Registration Hub

1. Child's birth certificate
2. Parent/guardian driver's license or state issued ID card
3. Student's immunization records (Along with any other important documentation from the physician)
4. Two proofs of residency less than 90 days old (*Envelopes with address are not accepted)
   1. utility bills (gas, electric, water, sewer)
   2. mortgage, rental or lease  agreement
   3. voter registration card
   4. change of address confirmation from the post office
5. Basic medical history and health information, including doctor and dentist contact information
6. Insurance information, including policy number and hospital preference
7. Know what school your child will be attending (Contact our transportation department- 330.456.2899- if you do not know.)
8. (If applicable) Custody papers- whole document with stamp date
9. (If applicable) Guardianship (Complete, current legal court documents)
10. (If applicable) Copy of current IEP and ETR
11. (If applicable) Previous school records/report cards

*If you have an existing student in our district, and already have a parent account in FinalForms, please login, and go to step 3.

We use the online service FinalForms for new student registration.

1. Click here to create an account
2. Under the Parent option, click *New Account
3. Fill out the form with your information, then click Register
4. Check your email, and Confirm your account
5. Create your new FinalForms password, then click Confirm Account

Click here to login to your account

1. Click Login (under the parent icon), then type in your username and password
2. Click the Add Student button (at the top right hand corner of your account window)
3. Select the school year
4. Fill out the basic student information (Elementary students do not need an email)
5. Click Create Student (at the bottom of the screen)
6. Choose the school your child will be attending (If you are unsure what school your child will be attending, please call our transportation department: 330.456.2899)
7. Fill out the school forms (Red tabs on the left of the screen)
   1. As you fill out each form, and click Submit Form, each tab will turn green signifying the individual form is complete
8. When all forms are complete, there will be a Forms Finished message

Final Steps: Upload child's documents

1. Return back to the first tab
2. Click Manage Documents section
3. Choose one document at a time to upload
   1. To upload your document, you will have to have a clear, scanned copy or picture of the documents on the device you are working on
   2. Please name the document appropriately, so office staff will know what is uploaded

  We'll be able to walk you through every step of registering your child; make sure to bring all of your paperwork.

Even though this is not required as part of the registration process, we believe this is our opportunity to add an extra special welcome for our future Leopards. We have community partners, staff, and fun take-home items to get your new kindergartner excited about their upcoming school year!
